Apply for Housing
You must complete the following steps to make a reservation at Crestview Apartments.
1. Complete the Application for Residence. Print and complete OR fill fields with pdf reader and save with a different document name.
2. Attach a copy of a valid government issued photo ID
3. Include the application fee. Make checks payable to Crestview Apartments, or text manager at 435-525-2354 for other payment methods.
Single Housing - $250 application fee. $200 will become the refundable security deposit at contract signing.
Family Housing - application equals one month's rent. 75% will become the refundable security deposit at contract signing.
Text manager at 435-525-2354 to arrange an appointment to sign the contract or to inform that the application is in the mail.
4. Submit: Email completed application and photo ID to crestviewhousing@yahoo.com and contact manager for electronic payment options. OR mail completed application, photo ID and check made payable to Crestview Apartments to:

Do not send the application to the address of the apartment you wish to live in.
5. Crestview Apartments will mail you the Rental Contract, which must be signed and returned within 7 days in order to guarantee your reservation. The school year contract period begins on the Friday before the first day of USU classes in August and ends on the last day of finals in May. Summer contracts begin on the Saturday before the first day of USU classes in May and end the last day of finals in August.
